What Do SNPs Consist Of?
SNPs consist of three types:
- Dual Eligible Special Needs Plan (D-SNP) – These plans are available to those who are eligible for both Medicare and Medicaid. D-SNPs optimize coordinated Medicare and Medicaid coverage.
- Chronic Special Needs Plan (C-SNP) – These plans are available to those who have disabling or chronic conditions, such as ESRD, diabetes, and heart failure.

What Do SNPs Cover?

How Do They Work?
In many cases, you will be required to choose a primary care physician and receive referrals from this physician when you need to see a specialist. SNPs may also require you to have a care coordinator.
